Contact with the care team
Each customer is assigned a care team. In your care team, familiar professionals will assist you to ensure smooth communication and strengthen the continuity of your care.
Care teams replace the Care Coordinators. If you wish to keep your current Care Coordinator, please bring it up during your next visit. The phone numbers of the Care Coordinators are automatically redirected to the phone numbers of the care teams until the end of the year.
How does it work?
Contact your care team when you need non-urgent care from a doctor, nurse, or physiotherapist. Care teams are available on weekdays from 8 AM to 4 PM. You can find your care team’s phone number in the list below, based on your municipality and the first letter of your surname.
